A Clinical Study of Total Gastrectoniy for Stomach Cancer


Abstract
Total gastrectomy is indicated in patients with gastric cancer when malignant lesion can-not be widely encompassed by subtotal radical gastrectomy.
The risk of operation has been reduced but the results are not still satisfactory than that of after subtotal gastrectomy. 53 cases of total gastrectomy were performed between May 1965 to April 1975 at Department of Surgery, College of Medicine, Seoul National University and analysed them. Following results were obtained.
1. During 10 years, subtotal radical gastrectomy was performed in 367 patients and 53
cases (13.5%) of them were performed total gastrectomy.
2. The most frequent age group was from 40 to 49 years in 18 cases and sex ratio was not significant, finding.
3. The location of tumor was body of the stomach (26 cases), fundus (13 cases) cardia (9 cases) and linitis plastica(4 cases) and 1 case of recurred cancer after subtotal gastrectomy.
4. End-to-side eophagojejunostomy with Braun anastomisis(40 cases), jejunal interposition (7 cases) and Roux-en-Y esophagojejunostomy(6 cases) carried as, restoration of alimentary tract continuity.
5. Operative mortality was 9.4% and 5-year survival rate was 12.5%.
